Athelas Rider Participant Packet (9 pages)

Initial agreement form which needs to be filled out prior to approval for sessions at Athelas. On this form parents and guaridians agree to abide by our rules and regulations.

• Annual Medical History form
• Personal Information form

Scholarship Application Form (1 page)

We encourage all participants to apply for the JoAnn Currie Scholarship once a year. This may help some with the expense of paying for some of the riding sessions.

If you would like to submit a testimonial the forms are located in the Athelas office at Northfield Farm. Please feel free to ask Cheryl or Anneliese for a form!
